Dear Residents, Families, Friends, and community members, 

Thank you for all of your support during this pandemic. We are thankful for our community during this time and look forward to welcoming everyone back as soon as possible. Today, it is my duty to inform you that as a result of our proactive and robust testing program, we have had a positive resident or staff member test result. This may have an impact on upcoming visits with your loved ones, so please contact the Facility for more information about our current vistiation policy.  

The Facility will continue to engage in COVID mitigation and control procedures, including increased testing and other recommendations by the CDC, CMS, ISDH or our local health department. The Facility is currently in Phase One of the CMS re-opening plan. This means that any allowed visitation will be outdoors only. 

Stay safe and again, thank you for being a part of our Facility community.

Dear Families, Residents, Staff and Community Members,

We continue with our routine testing plan to prevent the spread of COVID-19. These scheduled tests allow us to make real time decisions that directly impact our ability to fight this virus. It is my duty to report to you today that we have had additional positive results in the Facility. Our testing is increased and the impacted staff and/or residents have been removed from contact with residents who are testing negative. We continue to have visiting restrictions in place. Please call for more information regarding visitation with your loved one.

To our valued Community, 

I am writing today to update you that the Facility has just received additional  positive test results. The impacted staff members have been removed from resident contact and impacted residents are being quarantined and monitored at the Facility. The Facility is continuing to follow increased PPE and infection control procedures. At this time, we also continue to have restrictions on visitation to the Facility. If you have any questions, please do not hesitate to call the Facility.

To our Valued Community,

I am writing today to update you on the fact that our routine testing has revealed an additional resident positive test result. All positive residents are placed on infection control protocols and removed from contact with their peers. Residents are being closely monitored for symptoms and our routine testing continues. At this time, there are a number of mitigations in place, including increased PPE use, increased infection control protocols, and visitor restrictions. Please call me at the facility if you have any questions.

To our valued community: 

I am writing today to let you know that our most recent round of routine testing has revealed additional resident cases of COVID-19. The Facility continues to implement increased infection control protocols. This includes segregating positive residents from their peers and a continued restriction on visitation. To our Valued Community, 

I am writing today to inform you that our routine testing plan has revealed additional and staff and resident positive results. The impacted persons have been separated from their peers and staff have been removed from resident contact as per our guidance. We continue to have increased infection control and PPE use in the Facility. We are also routinely monitoring staff and residents for any symptoms so that we can quickly isolate and/or test for spread. There are currently visitor restrictions in place, but we ask that you contact the Facility if you wish to arrange a virtual visit with your loved one.
