Dear Residents, Families, Friends and Staff,
Our Facility has reason to suspect an acute gastroenteritis outbreak within our community.
Currently, we have no confirmed cases of any virus and are following the Centers for Disease
Control and Prevention (CDC) as well as the state and local health department guidance for best
practices and procedures to protect everyone’s health.
If you are experiencing symptoms, please inform Michele Schmidt, Director of Nursing by calling
847-299-5588 Ex:529 or by sending an email to don@generationsoakton.com. We ask that you
continue to follow the Facility’s infection control practices including the wearing of all required
PPE. At this time, we will be temporarily closing our dining rooms to avoid spread.
Please note that visitation guidelines remain the same and visitation is still allowed. If you aren’t
feeling well, please stay home and visit your loved one at another time.

Thank you for all of your support during this pandemic. We are thankful for our community during this time and look forward to welcoming everyone back as soon as possible. Today, it is my duty to inform you that as a result of our proactive and robust testing program, we have had a positive resident or staff member test result. This may have an impact on upcoming visits with your loved ones, so please contact the Facility for more information about our current vistiation policy.
The Facility will continue to engage in COVID mitigation and control procedures, including increased testing and other recommendations by the CDC, CMS, ISDH or our local health department. The Facility is currently in Phase One of the CMS re-opening plan. This means that any allowed visitation will be outdoors only.

All positive residents are placed on infection control protocols and removed from contact with their peers. Residents are being closely monitored for symptoms and our routine testing continues. At this time, there are a number of mitigations in place, including increased PPE use, increased infection control protocols, and visitor restrictions. I am writing today to inform you that our routine testing plan has revealed additional and staff and resident positive results. The impacted persons have been separated from their peers and staff have been removed from resident contact as per our guidance. We continue to have increased infection control and PPE use in the Facility. We are also routinely monitoring staff and residents for any symptoms so that we can quickly isolate and/or test for spread. There are currently visitor restrictions in place, but we ask that you contact the Facility if you wish to arrange a virtual visit with your loved one.

To our Generations at Oakton Pavillion Skilled Nursing Facility Family,
Our routine testing has revealed a staff member has tested positive for COVID. Currently, our Facility is closed to routine visits. If you believe you and your loved one qualify for a compassionate care visit, please contact us so that we can evaluate your situation and assist in any way possible.
We are testing all of our residents and staff on an increased basis, have increased the use of our PPE, and most importantly, have increased our efforts to vaccinate all of our residents, staff, and friends to help fight this virus. If you have decided you want the shot, please know that we are offering vaccines to our Facility community. We would be happy to assist you in scheduling this important care.

At this time, we are pausing visitation to conduct testing of the entire Facility. This staff member was removed from resident contact. We are working hard to ensure that the virus does not spread any further. We are aggressively testing all staff and all residents, regardless of vaccination status and we are urging all of our residents and families to consider getting the vaccine or the booster shot to help fight this virus.
