Generations at Oakton provides each resident an individualized care plan within days of admission. The care plan is reviewed and changed quarterly or as goals are achieved or problems arise. This planning is an important aspect of healing, rehabilitation, and our caring program. The plan is coordinated by an interdisciplinary team which has members from nursing, rehabilitation, social services, dietary, and activity departments. Residents and family members are encouraged to participate in the Care Plan process.
At Generations at Oakton, we provide complete rehabilitative care including physical, occupational, respiratory, and speech therapy. Therapy services are available seven days a week so residents can receive intensive therapy to improve their range of motion, coordination, balance, and ambulation abilities. With our restorative and retraining program, residents receive quality physical therapy to return to their optimal level of function.
